Guest Declaration
By signing below, I acknowledge and agree to the following:
I will comply with all rules and regulations of Burgundy Serviced Apartment at all times during my stay.
I will not engage in any unlawful, illegal, or criminal acts within the premises.
I will not disturb other guests or residents with excessive noise, especially during quiet hours (10:00 PM – 6:00 AM).
I am responsible for any damages caused to the property, furniture, or appliances during my stay.
I will not allow unauthorized persons to stay in the room or access the premises.
I will not use the room for any commercial, immoral, or illegal purposes.
I will follow all safety, security, and fire prevention guidelines provided by management.
I acknowledge that the swimming pool has no lifeguard on duty. I swim at my own risk and hold Burgundy Serviced Apartment harmless from any liability, claims, or legal charges arising from any injury, accident, or death that may occur in or around the pool area.
Violation of any of these terms may result in immediate termination of my stay without refund.
Data Privacy Act Consent
In accordance with the Data Privacy Act of 2012 (Republic Act No. 10173) and its Implementing Rules and Regulations, Burgundy McKinley Place collects and processes the personal information you provide in this form for the following purposes:
Security verification and identity authentication
Visitor, guest, and tenant management
Building administration and record-keeping
Compliance with legal and regulatory requirements
By signing and submitting this form, you:
